What Does the Case Management System Do?
Case management is traditionally practiced with telephones and paper notes.
This was state of the art for the 1970's but the 21st century needs a
better system. Too much time and effort has been wasted on
time consumptive communication.
Case managers were not given an easy way
to identify those patients who were in need of more help.
We augment the telephone with secure messaging and replace
loose scraps of paper with a smooth, integrated website that allows
case managers, care providers and patients to take notes and keep in unified
touch with each other.
Based on physician assessment, case managers place patients on treatment
pathways appropriate to their diagnosis and personal characteristics. These
pathways provide guidance with automated reminders and educational
information.
Ignored reminders, missed appointments or other telltales will alert a case
manager to a problem; moreover, patients may message
a case manager with specific concerns. With our system case managers spend
more time actually caring for their patients and less time on routine
drudgery.
